Patient Profile

This section guides you through the four critical forms used to complete a patient’s onboarding and case profile. Completing each ensures a holistic understanding of the patient’s background and care needs.

How to Navigate: #

Go to the Patient Overview section
Locate the patient whose profile you want to view.

Click the Ellipsis Icon
At the top-right corner, you’ll see the three vertical dots (⋮). This is your action menu.

Select “Profile” from the Dropdown
A dropdown will appear. Click “Profile” to instantly navigate to the patient’s full profile page.

Completion Status #

Card has a “COMPLETED” badge to indicate sections that have been filled. You can revisit or edit them anytime by clicking the “View Form” button.

Form Sections #

1️⃣ Personal Details #

Essential Info
Includes the patient’s:

  • Full name, contact number, and email
  • Date of birth, address
  • Presenting complaint (reason for visit)

Purpose: Helps identify the patient and understand why they are seeking care.

2️⃣ Case Details #

Clinical Background
Capture:

  • Medical history
  • Pain regions
  • Ongoing medications
  • Existing diagnoses

Purpose: Ensures accurate clinical understanding and customized treatment planning.

Note: This is important and necessary form to be filled for generating Patient case (ICF and SOAP).

3️⃣ Insurance Details #

Coverage Info
Record:

  • Insurance provider name
  • Policy number
  • Supporting documents (if needed)

Purpose: Smoothens billing and reimbursement processes.

4️⃣ Physician Details #

Referral Contacts
Add:

  • Referring or primary physician names
  • Their workplace and contact info

Purpose: Facilitates coordination with other healthcare providers when needed.

Completing all sections ensures a full patient profile is available for the care team. Always keep this information up to date for best results.
